Replacement handlebar stem needed

The handle bar stem on my Santana tandem, which I need to replace, measures 21mm dia.OD but the smallest local replacement is 22mm OD. The existing component is part of a combined h-bar/stem unit. I need to raise the bars by a further 12mm than is possible with a stem length of 15mm to avoid wrist ache.
Are 21mm OD stems of 27mm length still available or will I have to engineer a solution? I do have electric arc welding,brazing and bench drilling facilities and am happier working with steel than alloy.
Options here appear to be:
a) cut off existing stem just under the bars and weld/braze a steel bracket to take new high rise bars
b) cut off the bars and weld/braze new high rise bars on existing metal a) seems neater than b)
c) cut off stem as a) and press oversize stem of correct length over exist stem and secure with screws/braze
Has anyone overcome this problem or got any advice?
